Our carer's self-assessment is designed to help you think about your situation and consider what you need support within your role as a carer.
You should think about what works well in your life, how you currently manage day to day and what you would like to change. It may be helpful to do this with a family member, friend or someone else you trust.
You can complete the form for yourself, or on behalf of someone else.
If at any point you feel you need help with completing this assessment because you are having difficulty understanding or remembering the information or communicating your views, please contact us on 020 8891 7971.
Once you have submitted the information to us, someone will contact you about the information you have given and discuss what support may be available to help you. We will also talk to you about your emergency arrangements and can answer any questions you may have.
