Report a lost, stolen or damaged permit

If a permit is lost, stolen or damaged, the permit holder must:

• Complete a lost/stolen form (pdf, 98 KB)

Once we receive your form your permit will no longer be valid. If requested, a new permit will be issued for the unused period of the original permit.

There is a £10 fee payable for replacement permits, except when the permit was stolen and a police crime reference number is provided.
